A joke that has gone on way too long. They started out as a bunch of art students putting on a show. The music was just generic metal, the lyrics were funny but the whole point was the rediculous stage show with buckets of fake blood being sprayed around and absurd costumes. I saw them in 89 or so and thought "that was fun"
At some point people started taking the music seriously and the "band" continues on with only a percentage of the original tounge in cheek that they used to have.
That video was funny though and makes me think they still take themselves less seriously than I thought.

ahh, GWAR... back in the early 90's, they made a video for a song called "The Road Behind" that was the greatest metal ballad video ever made..

They really don't take themselves seriously at all. Their music is a fine blend of vulgar jokes and social commentary. One of the best things on earth is people watching at a GWAR show, because about 25% of the people there don't get the joke at all.
