When laying out enduro courses, it's not terribly unusual to cut off small bushes etc. At one a few years back, someone cut a small bush with a machete, leaving a pointed green stick about 4" long. A rider went down, the stick went through a vent hole in his chest protector and about 2" into his ribcage. 
Ouch.
