Near as I can tell, and it's hard because of the catastrophic damage, the tiny Allen head countersunk bolts (top left in the tableau of destruction) backed out of the cam actuator and then slammed into the front cam journal cap. This then jammed the gear, which ripped the teeth off, before the bolts tore or bent or broke out of the actuator. The bits of gear and bolts then went everywhere in the timing chains. The Bank 1 cams got out of timing and slammed into the valves, which then bent every valve and tore rocker arms apart and flung them everywhere. It also busted a hole in the timing cover and the bank 1 camshaft cover. Bits of metal and needle bearings everywhere.

Any time I see bolts loosen up like that, I assume that things were getting hammered by unusual shock loads/vibrations.  Like, say, being run out of oil so the actuators are empty and hammering around.

 

Fasteners don't just loosen themselves, after all.  

 

I see many of these engines with 150k+ and no problems.  The only ones I see issues with are the ones that are trained to run low on oil.

I'll make sure to tell the gentleman that when he comes down today to harangue me about why it isn't covered under the special coverage for timing chains. Especially because looking at the history, the last time it was in, the tech found it had 1(!) quart of oil in the pan and when he mentioned it to the owner, the guy said "I don't know why it would be low on oil. I do all my own oil changes and I always put 5 quarts of synthetic in." Well, 1) it leaks oil from everywhere but the radio knob and, 2) these take 6 quarts of oil

Once a week I have to take the wife to work to take care of some things she can’t do from home.  The very old building their offices are in has a terrible mold problem so until it gets taken care of her department has been moved to a makeshift office in the maintenance garage.  Some lockers and storage cabinets were rearranged to square off a space, electrical and computer lines run, and this is their new space:

The whole payroll/personnel dept.  There are eight of them working there inside the supply cage:

They are taking it very well considering they all used to have private offices and do to ongoing repairs their bathrooms are  in a trailer outside.

I'm pretty sure Hyundai-Kia makes their connecting rods out of papier-mache. 2013 Elantra with 70k miles. And I've seen a dozen of their engines at my work with the same failure this year alone. And I work for GM
