This is a proper lockout/tag out procedure for an old Bullard manual lathe that had the counterweight pulley bracket break off, right?

The power at the buss bar by the ceiling and at the electrical box on the side are both switched on.. it's been like this for 2 weeks..

As someone who has been at powerplants which were LOTO'ed in extremely formal and important ways, that gives me chills. Something like that at a plant could kill a dozen people pretty easily.

I don't mess with it. I don't have a LOTO tag and don't want one. I don't do it every day, and there are a hundred ways to do it wrong when the piping gets complicated.

Please, for the safety of the people you work with, don't just post pictures of this on teh Interwebs. Get a supervisor to PROPERLY LOTO that machine. Please.

Agree with others. Get some locks on the bus bar. I've been at places that have lost people due to poor machine guarding and have pulled a friend out of a lathe after getting his hand caught. I am slightly biased.

no one is going to be messing with that machine... us cnc guys hate it, and one of the 3 people that know how to use it retired 2 weeks ago.. as of right now, it's serving as a shelf to hold a speaker and they are talking about actually getting rid of it, which is what they should do with the other 4 that haven't been run at all in the 2.5 years i've been there..

Not sure what bad can come of that, but my dad worked with a guy at the steel mill that didnt lock out the coiler correctly. He got coiled up right along with the steel he was standing on when someone flipped the machine back on.

As for the OP, I agree with the other guys. LOTO that damn thing before somebody gets hurt. There was obviously some reason the tag was put on. If it was just make a funny picture, take it off. It wasn't funny. Otherwise, do it right.

Yea, lathes are no joke. I've got a pretty gnarly scar on my left bicep as the result of my shirt getting caught in one. Lucked out BIG TIME on that one, as I was probably a split second away from getting my arm amputated and/or being made into a pretzel.

Started a new thread on it.

Short answer is, not yet, but it's been less than 24 hours. I'm figuring out a game plan and taking a bit of time to let my feelings settle so I can make the best impression when I talk to potential employers. I'm a mixture of scared, despondent, and relieved for it to be done with. This was a case of "when" not "if" this was going to happen. I just wish I'd done more prep for it ahead of time.
