Last week my buddy and I were working on a Suburban, lady complained that there was an intermittent brake noise, sort of dragging, scraping sound.

Took it to the local dealer, who stated they couldn't find any problems with the brakes

We pulled the wheels, and found a small issue on the passenger side rear.

One of the old used up pads from the previous brake job had been left inside the rim, between the caliper and the mount, it was sitting there, sometimes against the rotor, sometimes against the heat shield.

How the hell did the tech not notice he had only removed 7 pads.

Because very few people are thorough.

Same reason my friends new Dana 44 hubs blew apart on the highway 15 min after we picked the Jeep up. None of the hub bolts were installed beyond finger tight.

Same reason the same Jeeps fiberglass hood flew up against the windshield, smashing the glass into my face at 40 mph. Hood latches not secured.

Same reason my oil fill cap was left off at Jiffy Lube and my motor had an oil bath!

That is the same reason I do all my own mechanical work.Now that I have to replace lost parts from a reputable body shop I guess I have to learn body work too.

Ahh, it happens. I know we all like to pretend that we are God's gift to the wrench, but I left the lug nuts lose on a customers Grand Cherokee once. We get in a hurry (it was half-past closing time for me) and have cranial flatulence.
As far as the brake bad, I have a hypothesis. When I am doing brakes at a shop I pull the old pads off and set them on the arm of the hoist as I'm working. I could see one dropping and falling inside the wheel that I've set on the floor under said arm. If I picked the wheel up and the pad didn't make noise I could see it happening. Seems like an odd set of circumstances, but it happens.
Or, if as I'm working on the car a old pad falls on the floor, I'm leaving it there 'till I'm done (as in, job is done, work order is handed in, car is in the back lot). Then I sweep up before the next job. So, if that pad fell into the wheel, it could happen.
