Well it's hard to judge size without a reference. However, it looks like a tool that you would place between the two beads of a tire. When you pull the lever it spreads the beads apart to give you room to scuff inside of tire so you can then apply a rubber patch.

That's not unreasonable. Not sure about actual size, just got the photo forwarded from a friend.
Pirate's guess seems good; all I could figure was that I'm pretty sure that going from upper pic to lower pic you get increasing mechanical advantage as it spreads, and is just about to cam over in the lower pic.

I noticed that too. It makes me think it's intended to keep whatever it is spread open when it cams over, allowing you to do whatever with both hands free.
I'll soon be 75 and I kind of recognized it as something they had in a gas station I work at when I was 14. A quick search on eBay, they have everything!
