When I was young, I racked up points on my license pretty quick and decided that I needed a fast car that flew under the radar. I bought this 1964 Acadian (A Canadian Nova) wagon, stuffed a 383 stroker in it, put the widest tires I could on the back and told the exhaust shop to make it quiet. I never got pulled over once in that car and never lost a race, either. I miss it, wish I had it back. To answer your question, the sound and the wheels are the giveaway for a sleeper.

My best friend's volvo is also pretty damn sleepy. Those in the know might see the Pegasus wheels, slight drop and eggcrate grill. It's got a full exhaust with giant quiet mufflers. Oh, did I mention a warmed over 302 with e303 and AFR heads connected to a T56 and a locking diff?

I would consider my ms6 a sleeper. No one really looks twice at it and it doesn't say Mazdaspeed anywhere on it. I figure as long as I keep the exhaust stock it should continue to be left alone by everyone. Eventually I'm going to update the wheels to 2nd gen ms3 wheels but that's pretty much it exterior wise.

When I took my Studebaker to it's first car show when it had the LS in it. I kept the hood on it because it was a little windy and I didn't want it to blow away. 

I sat behind the car and just watched people's reactions. About half the people would stop and look. Some would come around to the back, read the label, and then say to their buddy, "it's a Studebaker."

An hour or so went by and the wind died down, so I took the hood off. After I did this, everyone stopped. I heard many time, "That thing is a sleeper! It has an LS engine."

My car has shiny aftermarket wheels, it's lowered, has fiberglass fenders and hood, drag radials on the back. I agree with the previous statements. It has more to do with the stereotype of the car.

If I took the stickers off the windows, it would be a lot more of a sleeper.  

When I take it to the track, guys laugh at it, until I start it up to take it off the trailer.

My buddy has a pretty solid sleeper. Turbo 5.3 making ~700 rwhp. If it can manage to keep the axles in one piece it'll pull wheels. The rear tires are about the only giveaway that it 'might' be fast. Sidenote: the track would not let him run down the track with the Christmas tree on top.
