So I have lost coolant for the second time since owning 3PO. (System malfunction warning came on the dash). I found a badly rusted clamp on the lower hose going into the radiator the first time this happened. I don't see anything obvious now, but only took a quick look at I was on my way to work. I topped off the system at the reservoir, it was empty, let the pump circulate fluid for a few minutes, cleared the code, and got to work. Thoughts? Could be a leak only when under pressure, but I'm not seeing any telltale signs.

Here is a screenshot from Dr. Prius as well .

Anything stand out besides the low bars in the middle of the graph?

It could be a gasket that is failing. Iirc the gasket on the front cover gasket of my brother's 07 Highlander hybrid went at about 170,000 miles. Leaked slowly enough that it wasn't obviously leaving drips when parked, but enough for the CEL to pop one day.

Slow leak somewhere.  Just keep investigating until you find the leak ( and keep topping the fluid up until you find it)

There's nothing at all wrong from the Dr. Prius screenshot:

Circled in blue is the important number: blade voltage differential.

Dr. Prius EXAGGERATES it in the bar graph.  You have a very healthy pack & the other numbers look fine.

Thanks. I was wondering about that bar graph given the numbers are not far off. And yeah, keep it topped and keep looking is the plan for now. Haven't read of any obvious spots to look at.
