On both a 2000 4runner and 2011 Tacoma, one of the drivers side pads is about 2/3 the thickness of the other 3.  Is that normal?  Or is the caliper failing.

Not normal, fixed calipers, either the a pad is not moving freely in the caliper and not returning or a piston on that pad is not returning after braking. Is the pad wear even top to bottom or tapered? I have a 13 Tacoma w/68 K and pads are wearing evenly. Checked when I install and remove winter tires. In MN we have road salt 5-6 months of the year which results in corrosion and rust jacking of pads/pins which will cause that condition. If the piston dust boot is damaged, it will allow water/salt to enter and corrosion in the caliper or on piston will result.

On GX470's at least (which Im pretty sure share the caliper with some toyotas)  the inboard lower piston commonly siezes up.

Yeah, seized pistons on the Toyota 4pots are a known trouble area....my 2008 4Runner did it when it was two years old, my Sequioa I've replaced both of them by 100k miles.

Luckily, they're not expensive to replace, and one of the RockAuto aftermarkets even makes NEW replacements directly from the original molds for cheap (which I have been using on the Sequioa for 20k miles now).
