At what size should they of potential concern and would you recommend removal?

I just had one removed from my lower back. They cut out an eye shaped chunk of skin, so when it was sutured shut its just a line.
I had one on the right side of my nose by my eye. I had removed it by taking the laser mole treatment from a clinic in Toronto. It was just a small mole, so it healed pretty fast without leaving a big scar. In your case, if it's a big one and hurts, you are supposed to see a good doctor.

Get it checked NOW. Better to let the doc tell you to not worry about it.
My dermatologist has frozen off numerous moles from my body. He has also frozen/burned off quite a few other things (tags and warts). But, two turned out to be squamous cell and basal cell carcinoma.
I have one on my face that seemed like it quadrupled in size over night when I was a teenager. I think it's still there, hard to tell with the beard.
I had a rather large one on my chest remove after doctor saw it. Left an interesting scar for war stories.
Again, doctor recommended, removed from my cheek. insignificant scar.
Since we have had a rash of people getting moles removed to find they are melanoma and then all but 1 dying in 3 months or less (the other one lasted 8 months and died last Thursday), I'd say get naked in front of your Doctor and having them check/remove anything suspicious!
Now since all 3 people did this and the melanoma spread rapidly I'd suggest telling them you'd like them to double the radius they'd normally use. Melanoma is BAD stuff and you don't want to take chances with it

"Two converging, semi elliptical incisions," if I had a dollar for every time I dictated that; oh yeah, I do.
For pigmented lesions, it has to do with size, symmetry, variegation of color, signs of adjacent inflammation(redness) which may indicate rapid growth, change in size or color, there are a lot of things to consider.
