How about the freakish Orange Blossom Express? A tank turned into a monster truck that was stylized to look like a steam locomotive. Non-existent forward visibility and hideously poor ride quality (watch the dude's helmet get launched at 2:23) made it a failure.

That thing had 0 driver protection. I'm really surprised they didn't hit the kill when his helmet flew off.
