When we upgraded to 220, my father didn't like the way the main was fed into the meter (it was on the front of the house). So he decided to redo it. Hot, because there was no way to turn the main off because the transformer cabinet was locked.
Now, he was a very good amateur electrician, but he was not a pro. He got the wire rerouted successfully and was in the course of reconnecting it when he grounded the hot lead to the side of the box. There was a huge orange blare, the transformer gave off a giant buzz we could hear from 50 feet away, and the blade of a heavy 10" Craftsman screwdriver disappeared. Vaporized. Dad was left holding the slightly melted plastic handle.
He had minor pinpoint burns on his face. Later we looked, and the lens surfaces of his glasses were all pitted from tiny bits of molten screwdriver.
