I'm trying to find a cap, or plans to finish off the roof on my latest backyard project. 

I have not been able to find a cap or plans for one to finish off the roof. I'm open to metal or other options, and I can always paint it if required. 
 

Any suggestions on where to find one or plans to make one,

Where did you get the metal and ridge you have?  The supplier can't help?

I bought to roofing and ridge caps from Lowe's, so I have not had much luck getting help from them when I've been there (but that was evenings). I'll have to go over one morning next week when their pro desk is open to see if they can look something up.

That kind of stuff is usually field fabricated out of matching trim stock.

Look up cupola. Pricey, but you could cut one down?

Also, +1 on bending it out of the matching material. Do a cardboard pattern, and it shouldn't be too tough.

Maybe see if you have a metal roofing place nearby.

I don't think you are gonna find a pre-made one.  They wouldn't be standardized, because the pitch would change each time.  Like Duke said- they are usually field fabricated. 
 

If you had spent some time fussing with the hip roof ridge caps, you could have gotten them to overlap and pop riveted them together. 
 

Lowes won't be able to help you. 
 

Yes, you could modify a cupola.

You can make one. 
 

Picture a circle with a pie shaped slice taken out of it. This can easily be shaped into a cone.

Now do it with a square instead of a circle. Voila!

I figured that was the route I'd have to go, but was holding out a little hope that I could find a premade solution. 

I'll make a cardboard pattern and then transfer it to metal.
