I think it is a 5th wheel that has been modified. That hitch looks super sketchy.

FWIW, I spent the weekend sleeping in my parent's Scamp (older, regular sized).  They're not wide enough for anyone +6'0" to sleep straight-across.  Maybe the larger bed is wide enough for a diagonal situation....but definitely not if you put 2 people in the bed or one single person on the front-end nook.  It was better than sleeping on the ground, so zero complaints on my end. 

The hitch adapter might be factory or could be aftermarket.  Regardless these are highly desirable, the only 5th wheel towable by a half ton truck.
