We had one of those, a 155mm. Turns out the firing mech that screws into the nose of the shell was being shipped LOADED to the field. Set off early.
Imagine the poor UPS guy carrying a box of those around in his truck fully armed?

Some digging turned up this. This was a South Korean 155 test gun. It was loaded with a special test round of high explosive. That shell detonated in the breach as soon as the shot was fired. 5 injured, one killed.
They might want to work on their test procedures. I don't think I would want to be within a mile of it unless the bunker was well built.
My experience is that 90% of incidents are caused by human error and/or lack of maintenance.
